Serial troponin I measurement in ACS

🔴 Suspected ACS-এ Serial Troponin I করার নিয়ম: 🔷 যদি high-sensitivity Troponin-I (hs-cTnI) হয়: 📌 1st sample: 0 hour (presentation-এর সময়) 📌 2nd sample: 1 hour পরে Patient যদি observe zone-এ পড়ে, তাহলে 3 hours-এ আরও একটি sample প্রয়োজন হতে পারে। ESC assay-specific 0/1 h এবং 0/2 h pathways recommend করে। 🔷 যদি conventional (non-high-sensitivity) Troponin-I হয়: 📌 1st sample: 0 hour (presentation-এর সময়) 📌 2nd sample: 3–6 hours পরে, যদি initial result negative/inconclusive হয় এবং suspicion থেকে যায়। 🔴 Summary: 📌 Serial hs-Troponin: 0 and 1 hour. 📌 Conventional Troponin: 0 and 3–6 hours. Note: 📌 Unstable angina তে Ischemic ECG change থাকবে কিন্তু Trop I normal থাকবে। 📌 NSTEMI/STEMI তে Ischemic ECG change থাকবে এবং Trop I বাড়তি থাকবে। শুরুতে Normal থাকলেও পরে বাড়বে। Reference: 2023 ESC Guidelines for Acute Coronary Syndromes; ESC high-sensitivity cardiac troponin algorithms.
